NCQA Patient-Centered Medical Home

At HIMSS11, NCQA presented a session on the NCQA Patient-Centered Medical Home Standards and its alignment with meaningful use.  It also advocates the use of electronic health records in implementing its PCMH model and that it would assist providers in achieving meaningful use.  NCQA recently released its 2011 PCMH model as of January 31, 2011.

According to the NCQA presentation, there are presently over 1,600 PCMH sites; Pennsylvania, New York, Michigan, and Texas have the most PCMH sites.
